Most likely a bug in the IOS you are running. You may want to open a ticket with Cisco. They most likely recommend an upgrade.

I have a stack set up and running using three C2960X. I tried to do a "sh int trans det" to check the power on the transceivers and found all of the ports on switch #3 where missing although they appear to be functioning correctly in the stack. I also found them missing in the inventory. Is there any reason I wouldn't be able to see the transceivers on switch #3 in the inventory?
