    <description>&lt;P&gt;I am pretty sure this is browser related and issue of compatibility of later browser versions with earlier versions of ACS&lt;/P&gt;
&lt;P&gt;Do you have any patches installed with ACS 5.4? What browser and version are you using?&lt;/P&gt;
&lt;P&gt;hard to track to a specific CDETS but seems similar to the following fixed in 5.4 patch 1 that relates to issues with Access Policies (issue was resolved over 3 years ago)&lt;/P&gt;
&lt;P&gt;&lt;SPAN style="font-family: 'Calibri','sans-serif'; font-size: 11pt;"&gt;CSCuc13958&amp;nbsp; ACS5: Pages do not display correct when using FireFox version 15&lt;/SPAN&gt;&lt;/P&gt;
&lt;P&gt;I would recommend to install latest patch for ACS 5.4: ACS 5.4 patch 9&lt;SPAN style="font-family: 'Calibri','sans-serif'; font-size: 11pt;"&gt;&lt;/SPAN&gt;&lt;/P&gt;</description>

&lt;P&gt;I &lt;GS id="95d2aa7c-3d94-4d23-826a-118535af51ef" ginger_software_uiphraseguid="d51c30ae-0835-4366-aaf3-a3fe1d9e0835" class="GINGER_SOFTWARE_mark"&gt;always getting&lt;/GS&gt; "system failure error "on &lt;GS id="c4f427b8-5999-412d-8ecd-92299b7be3e7" ginger_software_uiphraseguid="d51c30ae-0835-4366-aaf3-a3fe1d9e0835" class="GINGER_SOFTWARE_mark"&gt;acs&lt;/GS&gt; access policies. &lt;GS id="ca81b54c-0ad1-4973-8f80-0f3c45eeed20" ginger_software_uiphraseguid="a9b96d29-e588-48da-be56-55590cd2858c" class="GINGER_SOFTWARE_mark"&gt;It s&lt;/GS&gt; a newly created virtual appliance and nothing configured. I have tried to reset server, but no luck. It asks me to save the changes whenever I'm on access policies indeed no&amp;nbsp;changes done, if I click save to rid off and it throws me the mentioned error.&amp;nbsp;&lt;/P&gt;
&lt;P&gt;My ACS version is 5.4 and I downloaded it from &lt;GS id="d8ad1819-cb55-496e-9970-6e7df020b502" ginger_software_uiphraseguid="140be736-503b-4691-a2fd-15599d8c1050" class="GINGER_SOFTWARE_mark"&gt;cisco&lt;/GS&gt;&amp;nbsp;software portal only.&lt;/P&gt;

&lt;P&gt;I believe you are hitting the following bug:&lt;/P&gt;
&lt;P&gt;&lt;A href="https://bst.cloudapps.cisco.com/bugsearch/bug/CSCud78248"&gt;https://bst.cloudapps.cisco.com/bugsearch/bug/CSCud78248&lt;/A&gt;&lt;/P&gt;
&lt;P&gt;There is a workaround listed and the issue was fixed in 5.5 release and later so if possible I would recommend upgrading &amp;nbsp;your system.&amp;nbsp;&lt;/P&gt;
