Has anybody got a working CSV file as below that they can send me so I can get the idea of exactly the format.

RDBMS synchronization import definitions are a listing of the action codes allowable in an accountActions file. The RDBMS Synchronization feature of CiscoSecure AccessControlServer (ACS) Appliance uses a comma-separated value (CSV) file named "accountActions" as input for automated or manual updates of the CiscoSecure user database. Each line in accountActions represents one action, with the exception of the first line, which is ignored during synchronization events. This permits the use of the first line of accountActions as field headers.
