RESPONSE:

Could be hitting defect on UseCase EQUALS GuestFlow. In other thread, it souynded like you were testing with ISE 2.3 P2. This was fixed in P3 (already at P4):

Attributes for the guest flow do not match in the Authorization Policy.

Yes, I am using the following

Version 2.3.0.298

Installed Patches 3

Do I need to move it to patch 4?

As the fixed versions shown are in 2, 3 and 4...

doesn't seem likely then. did you troublehshoot with tac? recommend looking at the ISE Guest Access Deployment Guide

If all working except matching on Guest Flow, then try recreating condition manually. If fails to match on condition but Live Log shows use case = guest flow, then need to work with TAC to verify if bug needs to be re-opened.
