Heres the run-down.

I have one Cisco 2504 Wireless Controller, and 25 AIR-CAP1602E-A-K9 running in lightweight mode.

Currently, everything is humming and working properly, but on occasion users cell phones will stay connected to the AP but would lose internet connection. Simply turning off and on the wifi on the phone will allow it to reconnect to the AP and then be allowed to send and receive internet traffic once again.

It happened to me recently, decided to not do anything on my phone and tried to find my phone's mac address from the logs in the Controller but couldn't find anything.

Does anyone have an idea of why this is occurring, and also, a potential fix?

Can you post your WLAN profile config (standard and advanced), and AireOS code release please?
There are few connectivity issues within some releases and I was suffering something similar in the past.
