CBW141ACM will not reach Mesh Stage 5, Solid Green

I have a very simple wireless network with two 240AC (wireless access points). I'm adding a 141ACM mesh extender, which is brand new. After plugging the 141ACM into an outlet, the LED light progresses from blinking red (Mesh Stage 1, starting mesh through searching for parent) to blinking red/amber/green (Mesh Stage 2, mesh AP image data) to blinking amber (Mesh Stage 3, mesh AP image data) to blinking green (Mesh Stage 4, mesh AP joined controller). According to the Cisco Business Access Point Administration Guide (ver. 10.4.1.0, pg. 118), blinking green means that the "Mesh AP joined controller." All this seems normal. But then the LED light does not reach solid green (Mesh Stage 5), but rather blinks red/amber/green and cycles through Mesh Stages 2, 3 and 4. It never reaches solid green (Mesh Stage 5), which means that "At least one client connected." It continuously cycles through Mesh Stages 2, 3 and 4. The Administrative Guide has a note for Mesh Stage 2 saying, "If mesh AP image version doesn't match the Primary AP version, the Mesh AP will begin upgrade, then reload. Reload will cycle through Primary Capable Stages 1-6, then Mesh stages 1 & 2." and a note for Mesh Stage 3 saying, "If mesh AP image version mismatch with the controller and started to upgrade image version and reload so stages 1-6, mesh stage 1,2 will be repeated." My laptop, and several other clients are connected to the WiFi network. When I log into the 240AC and click on the mesh extender, it says "AP having MAC Address ["...."] currently Not Joined with this Primary AP!!!" Under Wireless Settings>>Mesh, Mesh is "turned on." The firmware on my 240AC is 10.6.1.0 and shows as "Up-to-date." How do I get a solid green light on the mesh extender 141ACM?
