There is a 'demo' bundle located in the same section of CCO where you download code for the WLC.

You could always preview the webauth page and view source and copy that.

If you have 5508 WLC you can download it from here:

http://www.cisco.com/cisco/software/release.html?mdfid=282600534&release=1.0.2&softwareid=282791507

If other model, go to cisco download and choose the product to download the software for and from softwares to download for that product choose: Wireless Lan Controller Web Authentication Bundle

(It is same bundle under all WLCs but you may or may not have permission to download the software depending on what WLC model is listed in your contract).

Hi,

Go to cisco.com then click on download, wireless, wireless lan controller, 
stand alone controllers, select the model of WLC, and then look for 
Wireless Lan Controller Web Authentication Bundle 

Basically you can create a customized Web Authentication page, in order to do so 
you can follow the guidelines listed on the configuration guide below:
