WebEx Setup questions for Provisioning

Hello-

We are looking to develop a questionnaire as a prerequisite for provisioning of Web Ex (Meetings/Teams/Calling) services.  Has anyone seen an article or document that addresses all of the questions needed prior to provisioning of services?
