Hello,
It is possible you are exceeding the number of SPAN sessions we support. How many SPAN sessions do you have? We only support 4 unidirectional spans, and bi-directional counts as two. If you are exceeding this, I recommend you shut down the other sessions and stay within the limit of 4.

Hello,
What is the destination for your SPAN? The fault above would lead me to believe something is wrong with the dest you have chosen. What traffic are you trying to SPAN? What is your destination?
If it is a number of SPAN issues, we can verify the number of sessions in the fabric from an APIC via the following commands:
show monitor access session
show monitor fabric session
show monitor virtual session
show monitor tenant __(tenant name)__ session
Are the other two SPANs you have configured on the same leaf as the 3rd you are trying to deploy? As Michael mentioned, this would not be supported on the same leaf if they are bidirectional. Here is the scalability guide for your reference:

I am also attaching a PDF on configuring SPAN in the fabric that goes through how to configure it along with what traffic is captured by the different SPAN types
