Hello,
we would like to use a SP112. We registered this on a 3CX and connected a Brother fax.
Firmware version is 1.41SP1.
Once you have 1 fax sent, the SP112 hangs completely. It is no longer possible to access the web interface and sending further faxes is not possible.
After some time, the SP112 is no longer registered in the 3CX.
What can we do?

Yes, it's Voice Application log, but it looks incomplete. All messages have facility local0 and severity Information while full debug uses facilities local0,local1,local2,local3 and severity Debug (among others). Debug messages are important here.
OK, just blind shot for now. Can you
- disable T.38
- enable PCMA (a.k.a. aLaw) as the only codec allowed
- set RTP Packet Size to 0.020
and try again ? If it will work with T.38 disabled/PCMA enabled, re-enable T.38 and capture a failing session - all SIP packets from INVITE onward, all RTP packet and all log&debug messages. Use Wireshark, tcpdump or so to capture them (ask local network administrator for help in doubt).
It may (but may not) help to analyze issue in full.
Note that full log as well as full packet capture contain no passwords, but may contain other sensitive information - like phone numbers, IP addresses and so on.

If I set PCMA as the only Codec allowed, then i cant send a fax.

OK. We know the issue is related to the use of T.38 and we identified workaround - fax can be send via aLaw audio codec.
Now I need full packet capture of failing T.38 session - SIP packets starting at INVITE, RTP packets and even syslog/debug if possible. Saved in binary form by tcpdump, wireshark or so.
But one more blind shot - there has been firmware bugs related to T.38 and particular vendor/model end fax devices observer in the past. A suspect just another one. Note that firmware bugs can be reported to SMB support center only - even fully disclosed and analyzed bug posted here are ignored by Cisco.
By the way - despite you tried to remove called number from log, skilled eye can still read it.
