Cisco router model ISR4321 - SPA modules NIM-2T show "offline" after version upgrade

Something worse might happen.

This is working fine on this image loaded: 03.13.04.S - Extended Support Release 

When i tried to upgrade the version to isr4300-universalk9_npe.16.09.04.SPA.bin, hardware module will show "offline" even tried reload or re-seating the module

Post the complete output to the following commands:
1. sh version; and
2. sh logs

It was resolved when i try to upgrade again and capture the full log
