Had this issue and wanted to let you know how I fixed it with the scenario I was seeing. Seeing this message in my CX Cloud faults "AAA instability with server" condition %RADIUS-3-ALLDEADSERVER:.*. After running the below commands I was able to fix this issue. Hope this helps and is not a duplicate of another thread.
On my switch I ran the command 'show cts environment-data' and the return was:

Environment data is empty

Retry_timer (60 secs) is running
This is not how the output should be so I ran 'cts refresh environment-data' and the correct data appeared:
