Putting call on Hold fails

I'm having problem putting a call on hold in a specific scenario:
1. Agent is in call.
2. Someone dials directly to the agent extension (call fails for obvious reasons).
3. Now agent tries to put his active call on hold.
 
Result - there is an error in the CTIOS log and I don't get the eCallHeldEvent.
Here is what the ctios log says:

It reproduces on ouw own application (our app is implemented in Java) and with the Java test phones.
 
Does anyone know what this error means ?
Did anyone encounter this before ?
Currently we're working with the simulator since our environent is not up yet - Does anyone know if this has anything to with the problem ?
