Core Issue

The phone has reset itself rather than pick up a configuration about the user.

Resolution

The IP phone is encountering a Domain Name System (DNS) error. In the phone configuration found in Cisco CallManager Administration (System > Service), set the server to the IP address of the CallManager rather than a name.
