It's covered in the CAD Installation Guide. One tip to save yourself a lot of intermittent problems: configure Exchange exactly as it says, especially the various throttling policies. Many of the steps come from TAC cases where customers were having problems and that was the outcome after troubleshooting and working with Microsoft ISV support.
