Agents stuck in Not Ready

Some calls comes in and when the agent go offhook using Cisco Desktop(actualy its a CTI-Application which we developed) the call is suddenly ended. The status of the Agent switchs to "Not Ready" and the Cisco Desktop stuck in "off hook" mode. While The IP-Phone is already onhook. here what i got from traces;

15:38:17 pg1A-pim1 Trace: TelephonyDriver::ProcessRingTimeout: Agent failed to answer alerting line. AgentID=XXXX DeviceNetworkTargetID=5225

15:38:17 pg1A-pim1 Trace: CSTA BASE MSG, \B?

From the Cisco Desktop (or: our application):

-ErrorMessage:The call identifier is not valid.

OR:

The specified agent is not a party on the indicated call.

This happens not to all calls.
