Call result 2: Customer not contacted (BA Dialer error)
Call result 4: Customer not contacted (no ring-back)

Sounds likeyour agent does not have call waiting enabled and that is why the transfer is failing. However, a Dialer log with full tracingshould confirm this.

These indicate that the Dialer is having trouble dialing. I would recommend opening a case with Cisco. Include whether this is IPCC or some other ACD, what version of switch and IPCC. TAC will instruct you on what logs to collect if necessary.
To me, it sounds like a configuration error which should be relatively easy to clear up with proper diagnosis.
