I saw this discussion on the forums and checked everything they mentioned. It seems to be connection related but my user is plugged in with an ethernet cable. Windows firewall is disabled. Supervisor goes into her team view and then CSD resets after about a minute to 'Select Team'. I've had some intermittent issues with connectivity at this remote site but but all agents at the site are working. I haven't removed/re-added her to the teams supervisory role as others have done. I'm not sure why that would be necessary though.
