Is there a way to find in UCCX 9.x logs or reports whether a call was disconnected by the caller or the agent?

Yes, you will need to examine the MIVR logs.

CUCM CDRs will specify which side disconnected the call. If you have CDR logging enabled I would not recommend searching through the MIVR logs for this information.
