I have 7960 agent phones each setup with a single ICD extension and a single personal non-ICD extension.
Is there a requirement that the ICD extension must be the first line appearance on the Phone? i.e. assigned to the first button on the Phone. In other words, can I have the personal extension assigned to the top button and the ICD one assigned to one of the remaining buttons?

Hello,
ICD extension can be assigned to any of the buttons on the phones.Make sure callwaiting,callforwarding is turned off on this line.This is the only requirement.

It is better (in my opinion) to have their personal extension used as their ICD also. That way when they make outgoing calls from their personal line it will make them unavailble in ICD and back to available when they hang up.
