Can someone explain my licensing scenario? I thought that each premium license would give you two ports.

The maximum number of IVR port on the system is 400.
After 200 Agents you don't get any additional IVR ports

Graham is absolutely correct, even though you get 2 ports per 1 premium agent seat but it cannot exceed the hard coded limit of 400 ports. Check Server Capacities and Limits section of Design Guide below:
