Cisco Secure Email and DUO IDP/SAML

anyone out there have this working correctly? We have had a few cases with CIsco as well as Duo. Find it hard to believe this isn't better documented. DUO DAG is configured and we have tried multiple iterations of versions as well as modifications. SAML debugging shows all resources making it correctly but we continue to get :

Single Sign On Authentication Failed! Please contact your administrator.

on the  CSE side. 

Trying to get DUO team and CIsco team on same webex to review but was hoping someone out there had a valid config working.
