7916 - That key is not active here, when trying to use 2nd page

I am trying to setup a 7916 expansion module on a 7965.  I have created a phone button template, where the 1st two buttons are line, buttons 3-18 are speed dials, and buttons 19-30 are Service URL's.  The speed dials are showing up correctly on the test phone that I'm using, but when I try to access page 2 on the 7916, I get a message on the 7965 that says "That Key is Not Active Here."

Could anyone tell me why this could be happening?  The phone load on the 7965 is on SCCP45.8-4-1S.  The Expansion Module firmware is B016-1-0-4

I've finally found some documentation.

*****If you want to use the 2nd page of your 7916, make sure you have the Expansion Module set to 24 port instead of 12.  Duh......

I have the same issue, except with an extension mobility profile. Changing the module to 24 ports does not help, I still get "That key is not active here".

The product type in the the user device profile is 7965. Any suggestions?
