ad-hoc Conference Call Fails

I am working with some users and when they try to do an Ad-hoc conference using the software conference bridge the call drop then the conf. softkey is pressed to join the calls together. I have verified that the calls are using G.711 and that the CSS of the lines has access to the conference bridge. I have included the traces from Call Manager. The DN initiating the conf call is 4879.

Any help is greatly appreciated.

So, the DP assigned to the conference bridge is using Region that uses G711 to the phones?

Does the phone initiating the conference has a DP with MRGL that has access to this CFB?

Or is PSTN involved??
