ATA 190 fax configuration with SIP gateway

Looking for a solid configuration for ATA 190 Fax using T.38 with SIP gateway.  We have tried so many different options between CUCM and the IOS we still have issue randomly inbound and outbound.

Call  flow:

ATA 190 (T.38)-->SIP-->CUCM-->SIP-->IOS2921-->PRI-->PSTSN
