ATA192 bricked after factory reset?

I recently bought a little used ATA192 second hand.  I thought a factory reset was a good place to start so I applied power (but made no other connection).  The lights flashed as expected.  I then pressed the Reset button.  The expected sequence of lights did not appear and now as soon as power is applied the Phone 1, Phone 2 and Problem report lights are solidly on as per the picture.  The power LED is off and the unit is unresponsive to further operation of the reset button.
To me it seems "bricked".  Am I missing something obvious and is there any way out?

Thankyou for drawing my attention to the User Guide.  Unfortunately the combination of lights my device is showing is not described there - no mention of the Problem report LED being on continuously and the power LED off.  To me it remains a mystery!

To restart the ATA, use a paper clip or similar object to press this button briefly.

To restore the factory default settings, press and hold for about 10 seconds.

The LED behaviour for the factory reset:

  1. After you press and hold the button for about 10 seconds, the Power LED is fast flashing green.

  2. After the factory reset is performed successfully, all LEDs are fast flashing green for about 5 seconds.

I have done factory reset of ATA's, I dont remember as scenario where the ATA Bricked after doing a  factory reset. 

Connect the device to a network check if the device took an IP, access the ATA using IP. 

if nothing works, then the device might be faulty.

It may be that some one has tweeked the firmware but a FULL factory reset should be done as follows:

In the event that the ATA wont ‘Factory Re-set’ from either the re-set button or (if accessible) the web interface use the following method.

To reset the ATA192 to factory defaults follow these steps:

  1. Plug in power adapter.
  2. Plug in a phone to 'Phone 1′ port of the ATA192.
  3. Dial **** (Yes that is 4 stars)
  4. You should hear 'Configuration Option Menu'
  5. Dial 73738# (RESET#)
  6. Press 1 to confirm the reset.
  7. You will then hear a confirmation message

Default IP Address of the ATA192 is 192.168.15.1.

Assuming you're in the UK you will need to change the following settings:

UK Regional Settings:

Dial Tone: 350@-19,440@-22;10(*/0/1+2)
Second Dial Tone: 420@-19,520@-22;10(*/0/1+2)
Outside Dial Tone: 420@-22;10(*/0/1)

There’s a shortage of concise information and advice about these adapters, at least the setup for 3rd party SIP users. I believe there’s at least slight corruption in your helpful list of typical UK settings above, if it’s not too much trouble could please share it again? 
I also noticed there’s a puzzling long entry in the final entry field of the quick start section- with no explanation I can find in the documentation. Once again many thanks for taking the time to write your previous reply to the OP. 

Hi I’m sorry but I no longer have the information shown in the post, mainly because I no longer use an ATA as such. I now use a Fritzbox 7530 router which has a built in ATA – which incidentally I would highly recommend!

I found the UK details on the internet somewhere, (I guess it must still be out there somewhere) sadly I cannot recall where. As for the ‘puzzling long entry’ you refer to I assume you mean ‘When you have full access you will need to change’. I think that was badly copied and pasted from my then files and should have been placed just before ‘UK Regional Settings:’ in my post.
