Operators can’t place callers on hold using the attendant console or the hold button on the phone.

I would check the MRGL/MRG under the phone/device pool and see if there is a music-on-hold server assigned.

yes, make sure your Music-on-hold servers are registered. I am going to assume this is a UCM system and not CME since you mention attendant console.
