Attendant Console Standard - No Errors but No Calls

We have an attendant console standard 12 installation that was working for years but someone deleted the ac user account from CUCM. We've recreated it but are having issues.
CUCM cluster is 12.5.1.15900-66

The contacts sync at the bottom, but the call control window has a ? in the top left.

Calls to the extension don't show in the window and you can't make outbound calls from it.

The new ac user in CUCM is an application user.
The phone that has the extension on it is associated with the app user
The app user has all 11 roles that the install guide specifies assigned to it.
The app user and password have been put into the Options area of the app and into the TSP config.
I have used both IP addresses and FQDNs for the CUCM info in both the app and TSP config with the same results.

When you put in the extension to log into the app, it loads with no errors, just the X at the top left of the call control window (see screenshot).
