I'm experiencing call disconnect during transfer, when I enter C number its rings. then I press transfer its disconnects,
please guide to tshoot this

Start by mentioning what you're using and the detailed call flow, with just that info, the best you can get is: look either at traces and/or debugs to find root cause.

Please share the below information related to a failed scenario.
1. Is the transfer internal or external ?
2. IP phone model used ?
3. was this working before ?
4. Exact call flow
From the RTMT tool, we need the below logfor a failed scenario:
1. Detail CCM traces from all the nodes in the cluster.
Along with the traces, we need:.
1. calling party number
2. called party number
3. number to which the call is transferred
4. time stamp when the call is made
