call forwarding

i am facing one issue with one cisco ip phone 7960 regarding call forwarding .

my ccm server is 4.1.3 version

i am attaching the cdr error report and normal cdr report with this. normaly call type is simple in cdr . but in the error phone i am getting call type is forward how i can rectify this issue ?

What is the call flow of the call that you say has an error (forwarding).

Please include a CCM detailed trace of that call.
