Call Park Extension not showing on Parking Phone

Has anyone seen anything similar to this with basic call parking?  Any help is greatly appreciated.

CME version 8.8

Phone: CP8941

When the user parks a call, it parks but does not display the extension it is being parked to (Call Parked On: XXXX).
