Call Manager 4.2.3 SR2a - get a "Conference Bridge Unavailable" error message on the phone when they try to dial a meet-me number or initiate an Ad-Hoc Conference. It does not affect all phones on the system, only a few?

You need to check the MRGL for the affected device and ensure you have one assigned to it.
All Phones are assigned to the Conference MRGL - and the Device pool also points to this MRGL.
try restarting the IPVMS service or doing a sccp/no sccp if using HW CFB
Where is the IPVMS service?
Restarted service, I will make some test calls to double check
