After some time it goes back to firmware upgrade screen and comes back to Cisco boot screen. I did the factory reset mentioned in https://tools.cisco.com/bugsearch/bug/CSCef22943
Its still the same. Then I did the second FR with a different code 3 4 9 1 6 7 2 8 5 0 * #
metioned in the above link. The issue has become worse. i cannot see on the screen now. Its blank.
Can anyone help me with this?

Basically you bricked the phone to a no-firmware, or almost only bootstrap firmware. Generally this is not the end.
Basically you need to place a Good DHCP with option 150, and a TFTP server with all Config and firmware files needed from the phone, in order to get back it's Operating System.
My favorite way of doing this, is setup a SPAN port on the SW, and monitor the traffic the phone is sending with Wireshark, you can then see every DNS request and TFTP request the phone is doing, and try to get the files the phone is requesting.
On the community there are some document on how to upgrade a phone firmware using TFTPd32 those might come in handy.

The issue is resolved. The problem itself says that its related with the Firmware.
We have a upgrade process in pipeline at that time. We had CM 7.0 version and we migrated to 10.5 version.
Once we upgraded to CUCM 10.5, the phones started downloading new firware and registered.
