Cisco IP Phone obtain both DATA Vlan IP and Voice VLAN IP randomly

hello gents

I have CUOM installed to monitor our Cisco IP Telephony Environment

I noticed that some of IP phones receives DATA VLAN IP

in the attachment you can see lof from CUOM

Switches details

146.31.162.21 

WS-C3750G-24PS

C3750-IPBASE-M, Version 12.2(35)SE5

any ideas?

The VLAN is assign by the switch so it's where the problem could be.. Folow this guide:
