Cisco IP Phones: Wont hang up

I can't figure out where this is in the settings or if the phone is just dead.  I have a host of cisco IP Phones all 7900 series, 7941, 7942,7945,7965,7970, etc.  And everyonce in awhile they will stop hanging up, you have to press "end call" to hang up.  Once hung up the phone operates normally till the next call which at the end of you have to press "end call"

Is this a setting somewhere? Did the end user accidentally press some button? Or is this on their template in the call manager?  Or is the phone just malfunctioning and need to be replaced?

7962 phones are going off-hook and then on-hook 400 milliseconds later 

Symptom:

7962 phones are going off hook and then on hook 400 milliseconds later, which prevents the user from answering the call properly. This behavior can be seen in the SDI traces:

11:33:42.540 |StationInit: (0000030) OffHook.|1,100,49,1.584290

and then back on hook roughly 400 milliseconds later:

11:33:42.981 |StationInit: (0000030) OnHook.|1,100,49,1.584297

Conditions:

So far, this has been seen in IP phones starting with serial numbers FCH1450 and up to FCH1504
