Cisco/Polycom 7936 - tones not being "heard"

We have a 7936 conference room phone and have not changed any configuration settings with our UC 560 system lately. We've found that once a call is connected with this phone, the tones are not being "heard" by the other end when people are pressing buttons on the keypad. We can hear the tones on our end, though.

This issue is making it impossible to set up a phone conference with our conferencing service, or get through anything with a phone tree, including our own voice mail system.

I looked at the settings menu on the phone and don't see anything that looks like it might be causing this. Shoud I assume the phone is now defective, or is there something I can check either with the phone or phone system?
