Please help to answer my queries (Note:We have CUCM Ver11 with single cluster)
1. What are requirements need for soft IP phone configuration ?
2. Hard IP phone license is support for Soft Phones ?
3. License cost ?

Enhanced license will do for your CIPC. You can get the license/cost info here (Page3) Table2

Cisco IP Communicator it just another phone, so any license will do.
Depending on the features enabled on the phone/user that will dictate the type of license needed.
