Your right as standard in CME you can only do a 3 person ad-hoc conference. CCM supports ad-hoc and also meetme conferencing which is done in software. The meetme conferencing will allow you to support over 3 parties.
If you want more than 3 in CCME then you need to use hardware based conferencing which uses dsps so you need to have extra PVDM installed. You can then use meetme and ad-hoc conferencing for over 3 parties. Also im pretty sure you need CCME 4.1 to be able to use meetme

no, CME by default can only host 3-way party conferences. if you want more people u need to add a HW CFB.
CUCM has by default the 4 participant ad-hoc conference but you can change the service parameter for max ad-hoc conferences to increase that, just look for it under the CUCM service parameters
for CME:
Ad Hoc Conferencing
http://www.cisco.com/en/US/docs/voice_ip_comm/cucme/admin/configuration/guide/cmeconf.html#wp1010427
instructions on how to add a HW CFB are also in the same link
both systems can have more than 4 participants but for CME you need HW CFB and for CUCM increase the service parameter.
this topic has been discussed many times in the past, if you use the search NetPro feature you will find several threads on the same
