CUCM 12.5 is sending only enbloc to all destinations and if dial digit by digit it will reach only to third digit.

There's no way for anyone here to tell you what part of your configuration is the problem, only YOU who has access to everything in your server can know the answer by analyzing your dial plan and configuration.
Use DNA with the 3 digits you dialed and see what was matched.

In DNA enter the digits that is dialled, if it stops after 3 just enter those 3 digits and do the analyse.

DNA may? give result based on enbloc sending digits and all is fine,

Another question
Gateway Call Routing Information - Inbound Calls MGCP
Translation pattern does not work, added Calling Party Transform Mask is not being translated.
Other translation pattern same configuration are ok.
What could be the reason.

You don't give nearly enough information for anyone that do not have visibility of your system to help you. Please describe the call flow you have and what you have done up to this point and what your expected end result is. Then possibly we will be able to help you out. If possible add pictures of the configuration you have, with descriptive text so that we understand the context.

If you're doing off-hook dialing digit analysis is done digit by digit, if you dial the numbers on-hook and then press dial, they're all sent at once and digit analysis works with all the digits that are received. You'll need to look at your dial plan to understand why it's dialing when only 3 digits are received and fix your configuration/dial plan as necessary.

Can this be because we edited translation pattern to xxxx to fix issue with Calling Party Transformations which was showing incorrect.

sorry my mistake it was after 4th digit
