Hi Joe,
As I could verify, callerID is received by Gateway. Could you please collect the logs when callerID is not been received? Attached running-config and explain call flow too.
Calling Number=11999999999,(Calling Name=)(TON=Unknown, NPI=Unknown, Screening=Not Screened, Presentation=Allowed),
Called Number=5502(TON=Unknown, NPI=Unknown),
Calling Translated=FALSE, Subscriber Type Str=Unknown, FinalDestinationFlag=TRUE,
Incoming Dial-peer=10, Progress Indication=ORIGINATING SIDE IS NON ISDN(3), Calling IE Present=TRUE.
Error message "%VOICE_IEC-3-GW: CCAPI: " could be related from a Dialplan misconfig to IOS Bug. It's hard to tell only seeing the error message.

We don't know if there is any relationship, just told you some comment about two different issue, CallerID and error message: VOICE_IEC-3-GW: CCAPI. Still don't know if there are related.
