DX650 - Forget credential (user name and password) and screen lock PIN

Hi

I have CUCM 9.1 and ip DX650 ip phone SIPdx650.10-2-3-26

I need to reset the  screen lock PIN and the IP phone request the user name and pasword for Cisco user in other to reset that PIN

Where can I get these credential : user name and password ?

How can I reset those credential and screen lock PIN ?

 

Please help
