ephone-dn stops working after call-forward timeout (no dial tone)

I've got CUCME 14.1 (17.12.03) running on a 4321 in a lab environment. 

Everything works perfectly for the SIP phones as well as with the paging and conferencing ephone-dns. The only issue is that when an ephone-dn call-forward timeout expires (ephone-dn 7 in the attached config), the call is forwarded to voicemail properly. But then the ephones using that ephone-dn no longer have dial tone and cannot receive or place calls. In the attached config I'm using analog phones connected to a NIM-4FXSP. However, I experience the same issue with 7975 SCCP phones as well. 

Once I delete ephone-dn 7 and re-add it to the config, everything is working again. ephones using ephone-dn 7 will operate properly until the call-forward timeout is reached and then they are back to no dial tone and not operational. 

I tried some debugging but didn't find anything useful. 

Any ideas? Thanks.

I switched over to 17.09.05e and that fixed the issues I was having.

I forgot to mention that the ephones remain registered when in a failed state.
