Firmware reverting to old version

I posted a week or so ago about one of my phones randomly power cycling. It was determined that the issue was prob bc of old firmware, so I asked the network admin about upgrading the firmware. We looked into it and it appears as though the phone has reverted back to old firmware, and i don't know how or why.

In CM, the phone is listed with an up to date load -

Active Load IDSCCP45.9-1-1SR1S

However, on the IP page and under the debug alerts, it appears to have swithced to an older load:

Any ideas as to why it's happening? I'm not sure if it's more than just this phone or not.
