Is the call forward to an external number? cell phone?
Is the call forward/caller-id working when the call is forwarded to an internal phone?
If the call is forwarded to an external number, you can run a
debug isdn q931
on the gateway and verify if Callmanger is sending out the caller-id and the redirecting number.

This is a DS1 SVPRI connection.

Not sure about the SV part of SVPRI. Haven't had the opportunity to work with SVPRI.
I would try it to verify if it works or not.
Login to your gateway and
term mon
debug isdn q931
and make test call.
