IP phone 7962 receives IP address from data VLAN instead of voice VLAN

I connected 9 Cisco 7962 phones to a C3750G-24PS-S switch. Only a phone received IP address from voice VLAN. The rest of phones received IP addresses from data VLAN. When I run "show cdp neighbor", I only see a 7962 phone that has IP address from voice VLAN. Any idea why it happened. CDP is enabled on all ports.

Can you post the output of "sh run brief" ? (Making sure you strip out things like passwords, etc)
