I have uploaded music on hold and set it to a phone extension in phone configuration under listed field i.e.
1) User Hold MOH Audio Source
2) Network Hold MOH Audio Source
but it doesn't work. Please help!

Maybe, because the MoH is not getting selected?
MoH selection is not only based on the phone, who presses the "hold"-key:

Furthermore, have you uploaded the file to every node, where the voice media streaming application service is running?

In addition to what @b.winter said, if you have more than one MoH server you will need to upload the music (along with selecting an index number) on all MoH servers in your cluster. The device pressing Hold indicates which MoH index number is played, while the MRGL of the device being held indicates from which MoH server the music is played.

I don't know why it is still not working? Its seems moh is not playing still.
