Gents,
When i unplug and plug any ip phone direct connected to 3750 switch with IOS 3750-advipservicesk9-mz.122-46.se.bin, the phone get disconnected power.
To fix the issue, i have to do physical switch reset.
Where is the problem? is it on the switch IOS or something else? i recently did the F/W upgrade on the phones to the Cisco latest.

Update switch IOS and check port configuration.

When the error condition occurs, are you seeing anything logged to syslog or the local log on the switch? You may need to reconfigure logging to see log messages.
Can't really speculate further without more information.

I would recommand upgrade IOS to at least 12.4T and above
