Is it possible to register a phone at a remote site if there is a pf sense router with an open vpn site to site config on it and a small cisco switch?

If you are asking about register Cisco  phone on PF router, that you need to check with the Router provider.

if you are looking for a solution to register phones from a branch to HO CUCM/CME using VPN, we can register.

Are you asking if you can have a phone register across a VPN link to a remote site to like a CUCM at a central site? If that is the case then yes of course just watch your MTUs some devices have problems like the 8831 didn't fragment registration traffic.

I have call manager express but there is going to be know voice router on the remote site i was thinking pfsense firewall with site to site vpn and a poe switch 

would the phone register with the remote site?

Yes it can register to a remote CME as long as you provide the needed information to the phone to know where to register.
