Please consider the following example:

Sw is layer 2 switch
R is router (Call manager express)--->199.199.199.3
R2 default gateway----> 199.199.199.4
Assume sw and Router, ipphones are correctly configured and working properly.
Now someone say Jeff picks up ip phone-1 and dial 222 which is the extension of ip phone2 and Lee picks the ipphone-2.
Jeff says the first word " Hello"
Let focus on ip protocol here.
1)what Destination ip , will ipphone-1 use here to carry voice traffic " Hello" ?
