Module for 3745

My managers had been to a conference on voip hybrid solutions. The solution included a module that would accept pots lines and would send voice traffic over them if the primary t-1 failed. The module I have found( EM-HDA-4FXO)

appears to be for phones to plug into due to the rj-21 interface. Does anyone know of the module that will allow up to 4 pots lines to plug into a rj-11 or rj-45 type interface. Thanks.

NM-HD-1V support VIC2-4FXO, it will give you 4 analog lines.
